The Finance Committee has developed an electronic Google Form for submitting check requests. Best practice is to make check requests from your PTA supported email address. We are hopeful this will work for all of you.

Please prepare your request ahead of time as all fields are required for submission. Each Officer/ Committee is responsible for ensuring the check request adheres to the approved budget line item under their responsibility. To ensure compliant data we ask for:

  • Supporting receipt(s)
  • Documentation of the officer/ committee chair providing approval for the expenditure. This can be written, email or text message screenshot.

NOTE: The system allows a maximum upload of 10 receipt files per payee. Both receipts and approval can be uploaded as .pdf or image files with the Google Form:
