The Finance Committee has developed an electronic Google Form for submitting check requests. Best practice is to make check requests from your PTA @supportabernethy.org supported email address. We are hopeful this will work for all of you.
Please prepare your request ahead of time as all fields are required for submission. Each Officer/ Committee is responsible for ensuring the check request adheres to the approved budget line item under their responsibility. To ensure compliant data we ask for:
- Supporting receipt(s)
- Documentation of the officer/ committee chair providing approval for the expenditure. This can be written, email or text message screenshot.
NOTE: The system allows a maximum upload of 10 receipt files per payee. Both receipts and approval can be uploaded as .pdf or image files with the Google Form:
